is looking for a Nurse Practitioner (NP)

Nurse Practitioners are responsible for the provision of high quality care services for the THCC Family Health Team (FHT), and play a key role in achieving its vision and mission.Here's how:
As part of the team, you'll be responsible for the provision of family health nursing care to the population of patients seen in the family practice service and requires a thorough knowledge of primary care nursing within the scope of the Nurse Practitioner as determined by the College and Association of Registered Nurses of Alberta (CARNA) and the College of Registered Nurses of British Columbia (CRNBC).
Areas of focus include: assessing care needs on intake, triage and navigation to appropriate services, disease prevention and health promotion, travel medicine, chronic and complex disease care management, collaboration with the physician and the interdisciplinary team. The FNP collaborates with the interdisciplinary team to optimize patient engagement and care outcomes. The role also requires a high level of interpersonal skills to interact with patients and their families in a holistic and compassionate manner.
Day to day responsibilities include:
•Individual patient assessment and treatment - health assessment, immunizations, ordering laboratory tests, diagnosis, treating illness, prescribing medications, procedures, regular patient follow up to promote health and counsel on disease preventionQualifications:You're the missing piece of the puzzle with:
•Creating patient plans in follow up to assessments
•Making internal and external referrals as appropriate
•Coordinating appropriate diagnostic tests and procedures independently and/or in consultation with family physicians
•Participating as a clinical team member for complex and specialized care, including but not limited to, prenatal care, chronic disease, child and young adult health and wellness
•Engaging in networking, conferences, and presentations both as an ambassador of TELUS Health and as a knowledgeable clinician
•Maintaining strong knowledge of community resources in order to provide the patient with information and care resources outside of TELUS Health Care Centres
•Masters of Nursing - Nurse Practitioner, post-masters diploma Working Conditions/Physical Requirements:
•CRNBC membership in good standing
•CPR and ACLS (AB ONLY)
•Knowledge and experience in primary care
•Current experience in chronic disease management preferred
•Advanced clinical nursing skills within the scope of a FNP; additional training relevant to practice is an asset
•Skilled in program planning, implementation and evaluation
•Ability to take initiative and demonstrate proactive/preventative care
•Exceptional organizational and personal skills, positive and results focused attitude, and excellent written and oral communication skills
•Computer competency and familiarity with an EMR
•Valid driver's license and ability to travel - may need to travel between sites
•You will be required to work in a multi-disciplinary clinical setting A bit about us
•Typically, you would work office hours however; some flexibility may be required to accommodate service requirements for the operation
•You will be working in an office environment, which may require extended periods of time completing computer related work
•You will work an eight-hour day with an additional minimum of half hour for lunch
•You may be required to travel for meetings and centre support
